Maybe the pre-exam filters like the old woman with the wife/girlfriend question counts as intellectual but she's not a guaranteed encounter as people make it to the exam site. The point is he had already reached the final part of his own exam and there's no real reason to have him run the whole thing again just to prove he's still qualified. And that years exam not having any other notable participants was already established because we're told some years nobody passes the exam and other years you get freak occurrences like Gon's where a bunch of people pass.

The few tests of intelligence came because of the proctor and not because of his oversight. You do remember it's a plot point that Netero's exams are uneven and criticized by some high ranking figures within the association including Cheadle, the current chairwoman right? His exams were lopsided and also let in total psychos like Hisoka or people just using the license for other work like Illumi for no real benefit to the association.

It got faster while operating on Komugi because she was in a different state of mind and nothing says she wouldn't have gotten to the point of rapid to near instant surgery given time. Conjuring a replacement bodypart however, as I agree that's probably what archangel's breath does, is an instant action backed by 11 people and a ton of restrictions.
