I've got literally almost every single doctor who has worked on me or is working on me, the gut isn't the problem as much as the adrenals or cytomas in the neck, also possibly addisons ontop but they aren't sure yet part of the next batch of testing.
If I even get to that phase, they tried ruling out lymphoma though never confirmed it's likely cytomas in my neck, had like 6 biopsies this year and multiple isotopes put in me last 3 years and so many ct scans I lost count, only to find out each time I was going into metabolic acidosis

It can cycle a lot what I have is likely pheochromacytoma or paragolioma which is the same as pheochromacytoma but instead of the tumors being on the kidneys they are in the neck along your veins and CNS.
They can do a 24 hour urine test and will show them the hormones spiked for the tumors, which I haven't done the 24 hour but I did the initial which flagged for cytomas so now I need the 24 hour to tell them where.
Then probably more MRI's to see them, then surgery to remove them, problem is they are likely in my neck considering the lymph nodes, and no surgeon in town will touch me because the neck surgery is almost always fatal.

In Germany you don't even need to declare crypto gains in your tax report if you held for more than one year when you sold. I can withdraw 10 million to my bank account and if I hels for longer than a year, it will not go to the Tax report. If the bank asks to prove you held that long, just show them the two transactions. Actual MD tag here. I'm guessing you live in a rural area because of the way you talk about medicine and what you have been tested for. Neck surgeries are extremely routine if you go to literally any referral center (aka a hospital with a residency program for otolaryngology), so much so that the easy people do thyroidectomies now is through their fucking armpit because patients don't like having a smiley scar on their neck. Not fatal, in fact it's very weird of people even have deficits from them nowadays. Because you aren't fucking with the recurrent laryngeal nerve, you are in an even better situation.

If you are a middle aged male I'd say you probably haven't been properly screened for Whipple's disease (because despite the fact it's on the boards everyone giving forgets about it ten minutes later, and pathologists tend not to look for it because it requires doing a PAS stain on duodenal biopsies, which as a true blue GI pathologist, I can swear to you is not done routinely). But it's a bacterial infection that's hard to get rid of and can cause both gut and cardiac issues. It can also cause a patchy lymphadenopathy which can be confused by primary care docs for a neoplasm.

Also depending on what your description of white count is, you might have cyclical neutrophilic fevers, if you've had exposure to farm animals brucellosis is another one of those "not typically screened for and not tested for unless specifically requested" kinds of infections that can be quite debilitating but missed by the routine micro panels done looking for more common infections.

Yeah I got an infectious disease doctor doing a workup for that, and the pathologists I don't even believe have stained for amyloids when I had irregular kappa light chains and a borderline ratio, alpha 2 protein elevated for 3 years, no M band, two hemotologists have barred referring for surgery either they know something or not, the test for cytomas in my urine flagged for it, I suspect that's going to be it but they will still do further infectious disease testing incase.

But 6 years of swollen lymph nodes in the neck and groin.

I meant rural because only really old doctors tell people that neck dissections are super dangerous because they certainly used to be, but people just don't really die from that kind of shit anymore. And given all the panels you have had my assumption was you are being treated like you spend a lot of time out in the country.

Could be a lot of shit, infectious, some weird auto immune thing, depending on what/how you have been treating it it could be iatrogenic and just got caught in the spiral that happens sometimes when people start out with one symptom and then end up getting medicalized so much they can't tell the new side effects from the old symptoms.

Amyloid is pretty easy to see because it creates big amorphous blobs of pink on routine stains so unless the pathologist is particularly arrogant they usually just throw a congo red on it if the clinician asks them to rule out amyloid on the requisition.

Sorry it's been a headache. My only advice is to remember that healthy people have up days and bad days too so don't get too caught up in trying to generate a heuristic for patterning your symptoms. I've seen too many people fall down a rabbit hole on that one and they start living and dying based on lab results that in a vacuum mean... Well nothing. Like kappa/lambda ratios, absolutely meaningless unless the patient has an established hematologic malignancy or they are SIGNIFICANTLY out of whack (like 20:1 or greater).

Makes sense, and yes you're right there though my symptoms have been pretty consistent last 4 years, major neck pain/tingling pain in the lymph node/glands in throat, when worst severe migraines as well, with what I assumed was spleen pain on the left flank, GI saw gasritis but was inactive and the pains always there, now seemingly same pain in the right flank, I presume this is kidney because all ultrasounds/cts showed no abnormalities on the organs besides lungs but I was a long time smoker so that goes hand in hand.

Before I do any surgeries I need confirmation on what they want to rip out, lymph node biopsy showed cd43 and cd5 proliferation 46% in B cells.
Bone marrow showed nonspecific immunophentypic abnormalities, which is just making doctors heads scratch there, but I'm pretty sure it's hormonal based since prednisone seems to reset my system a bit.
