If they are "restricted anorexic", ie. they don't exercise, just eat less and less, they usually lose all their muscle, but their body fat can be pretty normal or even high in some cases. The muscle mass and basal metabolic rate will be hilariously low (I read stuff about low 700s kcal per day). Then they baloon up when the get refed. Basically, the yo-yo effect on steroids.The actual lean anorexics are people who exercise/move all day. Like, 1000-2000 kcal worth of exercise PER DAY. Those people end up with an almost normal BMR, almost normal muscle mass and like 5% body fat. Which isn't exactly healthy either.
