Would've it be much different from Medieval European kingdoms?

Perhaps it would follow the Byzantine scenario with semi-feudal monarchy. One thing I know for sure - Karl der Gross would never be titled by Pope, and there wouldn't be any cringe German cargo-cultism. Also, perhaps some severe rivalry with Byzantium, including the religious question.
