It depends on the person. Typically we tailor it to the individual so that it looks like an accident. Does the person drink heavily? Alcohol poisoning. Do they take illicit drugs? Fentanyl or heroine overdose. Having a history of depression makes thing easy for us, since basically any form of death can be a suicide. This is just for stuff in stable countries, though. Most assassinations are in unstable countries, where a kill team can just shoot the HVT.

I think it's the exact opposite. It's definitely not a legend, he really did indeed hatch all kinds of crazy assassination plots. We just know about the Castro ones and the time he went to poison some dude in Africa with a tube of toothpaste (but did not follow through). Imagine how many other successful ones he was behind. The only reason why we know his name is because the CIA forgot about a box of documents at UCLA related to MKULTRA, and that was only know about the LSD portion and contained a handful of other sub-project names with no context. Pretty sure they didn't want the Church Committee to find about any of this, and pretty sure Gottlieb's CIA fiefdom was totally separate from anything ran by Richard Bissell. I do know he was apparently given a tremendous amount of latitude like Bissell, but somehow MKULTRA didn't end his career like Bay of Pigs did for Bissell.

Actually read a whole book on Special Activities not too long ago, and the answer was more or less just proxies. Like, 90% of the time they'd have someone else do it. Generally if you're the kind of guy that the CIA wants to kill, you've already got some people with an ax to grind against you anyways that just need the tools. Wasn't really until Afghanistan that the CIA did a lot of in-house killing, at least that they were willing to admit to.
