The risk of two healthy people producing a child with a serious genetic defect is about 2-4%. For incestuous couples, like say, first cousins, it's about 6-7%. That seems high until you consider that the 2-4% assumes a woman 16-24. A woman in her 40s has a 5-10+% chance of defects, with down syndrome especially jumping to a 1 in 30 shot from a 1 in 1250 if she's 16-24.
