They believe that the Emperor is God and that he is looking out for their best interests and what he requires in return is faith and service. It's pretty simple to see where their sense of hope comes from when there are actual real manifestations of their faith happening during their lifetimes. The Living saints and other people and situations prove on the regular that the God Emperor is actively effecting reality which if you believe he is God then you would be very motivated and hopeful that their is progress being made in the fight against the ruinous powers and the xeno scourge.

Anesthesia however I do worry about breaking continuity. I'm not a medical man so I'd have to really look into how quiet things get upstairs to make a judgement but thinking back to some instances in my childhood I really do wonder if that entity ceased existence or not as a result of anesthesia. Two biggest problems with brain torture jar is 1) at any point the brain can just die from shock and 2) there are neurons in other organs such as the heart and the stomach. Really point 1 is the main issue, the brain not having a support structure also means it'll necrose significantly quicker, every pain signal you send it will tire out every cell and rapidly kill them without true circulatory support.
