I'm surprised how few games let you do this.
The only 2 I know of are dark souls 1 (i think all npcs are killable?) and tes 3 morrowind.

Even the original demon's souls didn't let you kill the maiden in black so it had that tes 4 oblivion onwards type of "this npc has a crown because they're integral to progress the game so we can't let you kill them" bullshit they pulled, and I know she's immortal in lore and you can kill her as one of the endings but still that doesn't count.
And future souls games also pulled the same stuff, Sekiro just outright doesn't even let you attack any single NPC at all.

It's amazing how rare it is to find and how much of a cool factor it added to souls 1 where you could make a comfy places as desolate as you wanted by just murder hobo-ing everyone, it truly made that game feel special.

Not OP, but I think you can dash to the stairs leading down to the bonfire, wait out the fire breath, run back up to bait the Drake to come down, then run past it to the bonfire. At which point it flies away until you reload.

The rest, sure, but they're all part of a boss spectacle, not that it excuses them.
There are no moments where you're exploring and the game suddenly kills you outright.

This is actually EXTREMELY annoying. I killed most of the key NPCs by mistake. The reason it's not "immersive" to be able to kill NPCs because the controls in the game are based around combat. If you press a single button you slash someone with a sword by accident. This completely ruins the immersion for me. In a "realistic" game world (ignoring the fantasy stuff that comes with the suspension of disbelief yadda yadda), you shouldn't walk around afraid of accidentaly unsheathing a blade and murdering someone in cold blood out of nowhere.
It's the same thing with Red Dead Redemption, another garbage game. Not killing any civilian or running over them with a horse by mistake is nearly impossible. I honestly prefer games like Yakuza where you're locked into moralfag mode and don't have to worry 24/7 about comitting mass murder because your finger slipped on the wrong button.

Like I said ds1 and morrowind are the only 2 games I can think of where you can kill everyone, and in the latter completely fuck the game up and softlock yourself.

That literally can't happenm, you haven't played the game.
You need to hit multiple times or do enough damage above a certain percentage of their total hp, which is usually around 300+
Fists do like 2 damage, so you'd need to hit them 3 times or have a damage cheat.

This is shit. It's no adventure running around trying to find where to go once you've experienced everything the area has to offer. This doesn't mean you get quest arrows and map markers everywhere. It should just let you know, or guide, where the unexplored area begins. Either by giving a rough compass direction, having NPCs explain how to get there or have something like enemies coming from that particular direction and leaving a trail.

No one feels good getting lost and having nothing new to explore or experience.
