There are bonus damage while in stealth perks at work, including one from stealthrunner which boosts melee damage from stealth even further. I'd assume those help boost the output. It's great, I'm still working out if I should swap it for the sniper or the knife. Tried knife now, but I kinda miss it. It's less risky than silenced pistol for one taps from a distance. Or being able to throw it at elites without instantly getting detected since they need about 3 chest throws to die. The sniper is fun, but the usecases are fairly slim. I can still grab it from the trunk of my car if I need it for a gig.

It's rarely completely bloodless but it happens here and there.
