There's mods for addictions, wear and tear, buffs and debuffs based on arousal, mods that comment on if you have had sex or not, etc. With oStim you have sex on a better engine but there really isn't much after. Fertility Mode+ exists, I think love sickness does too, but you can set oCum decals to basically stay on until you bathe and nobody cares if you walk around with it. You have zero consequence for your actions since nothing will trigger oStim enough to have to worry about things like pregnancies or being weighed down
