It's using a drawing program that allows for immense amounts of brush sensitivity in response to your pen (on an ipad esque device or tablet doesn't matter) [not photoshop for example where brush sensitivity is a bitch you'll have to manually change it a lot],, or it is literally going line by line and changing the brush size as needed. Manga is heavy on brush work, so when you're in a manga illustration mindset you're paying more attention to line work, and keeping in mind you will not be coloring it. It's half mindset and knowing what you need. Japanese pens in specific come out really inky it's like you're constantly doing calligraphy with them, which makes sense given how you have to write their language (changing an entire word based on one tiny ass dot). If you use even a basic uniball pen you'll see what I mean. You get linework and brush strokes with those pens that aren't even possible with other types of pens, and it seems like that shit translates over to Asian digital art (also a lot of manga illustrators will use traditional art elements at time).
