Tomboys used to be much more common because they could always find a place with the boys. But feminine boys have always been too timid to group with other boys and too male to fit in with the girls (because boys have cooties). This mostly continues through life, not that GD hasn't already taken a hold. The dichotomy is essentially transition or repress yourself until you die.
