inbreeding causes harm in two ways, first if there are bad recessive genes, they cluster and can create big health problems.
Second, homozygoty, instead of 2 different chromosome strands, you have 2 copies of the same one. That's not necessarily bad but it can be, it all depends on the quality of the chromosomes that stay. they cant be missing anything or any protein because theres no backup.

allowing for incest allows the bad gene to cluster together into very unhealthy individual, it reduces the number of hidden bad recessive genes, and increases the amount of expressed bad recessive genes. which in turn gets them out of the genepool faster.

same for homozygoty, those who get duplicate chromosomes of poor quality will die out. Leaving behind only those with duplicate chromosomes that work.

basically incest disproportionally gets rid of bad genes by killing the people affected by the diseases and removing the safety net of a double chromsome setup. Though to get the benefits you would need to make incest the norm for a very long time.
