I'm disabled and mentally ill, and i can tell you seeing a character like me shoehorned into a show does not feel good or fill me with joy, it makes me roll my eyes so hard it hurts. Being mentally ill or disabled is not fun, and it doesn't make for fun characters.

Berserk, kinda. AOT has a bunch of characters that suffer, but mostly towards the end. Reiner being one of the most tortured ones. Tojima Wants to Be a Kamen Rider kinda has those elements. It's filled with a bunch of fan service shots, so maybe it's kinda uncomfortable to watch. Anime isn't a good media if you're looking for those types of stories. They mostly center about self inserts and power fantasies.

Boku no Pico hits a little too close to home at times, but you did ask for a hated outcast suffering main character.
I can't believe no one recommended watamote yet. It was literally exactly my high school experience. It captures the feeling of being an outcast and hyper analyzing any bit of interaction you get because you just are so clueless and lonely. Slow start is also a really good anime. It's about a girl that had to delay starting high school because she was sick. She feels extremely insecure. I felt the same way when I transferred from private school to public just lonely and strange. Trigger warning though she does get friends.
Oh shit also 3 gatsu no lion. It's about a guy that is a depressed orphan and had a traumatic childhood.

I recommend It's always sunny in Philadelphia season 4 episode 9, I find Charlie so relatable in here. When your life is so horrible you do seemingly perplexing things for the other's point of view but they don't think it be like it is
