I would tell them it's okay to be gay and that they need to get actual therapy for their internalized self hatred rather than engaging in extreme sex acts in a misguided attempt at destroying or overpowering the part of them that they want to kill off, which it would appear to be their masculine identity, which could be for any number of reasons they're only really going to be able to identify and work through with the help of a professional in a closed setting. They don't have to hate themselves for being gay or for being a bottom or for being masc or for wanting to be or enjoying being femme or a masochist, but they do need to approach it in a much healthier manner and they need to learn to love themselves as they are. And wild as it may be to say this you can still be all of those things and do all of those things and still be largely conservative in mindset and political beliefs, even if the current party zeitgeist rejects who and what you are entirely in what is also a misguided attempt and destroying the part of themselves that they hate which they should really just embrace and understand and make peace with.

Alternatively if they get uppity with me I call them a faggot and bully them into furthering their descent into madness and pain so that they will eventually one day break and realize I was correct or they'll somehow manage to get wrongholed into happiness through ego death and radical acceptance of who and what they are. Those are basically their most realistic options. The former is a lot healthier but a lot harder, the latter is incredibly unhealthy but much easier, and the choice is ultimately theirs to make. And basically all of this advice applies to those afflicted with autogynephillia of the transvestistic nature as well as to quite a few that are transexual, although not all, because some trans people are actually doing it for the right reasons and are valid and reasonable people for having done so.
