Yes i also do that, for example i always regulate the initial npcs disposition based on a appearance (how handsome or not the character is, how presents himself, the clothes, etc...), but what i mean with mechanical way is having game effects tied to appearance, like for example a situational feat/trait that mesmerizes (stun) the npc for a moment after a successful persuade or something similar.

Ugly people don't exist in most games I run except as antagonists. There was one player who always wanted to play hideous freaks but I stopped inviting him to play after a while. That said I also rarely involve attractiveness as a mechanic unless the system itself supports it. The world is just full of beautiful anime white people.

We had fun, in fact, thanks to a rule related to character point distribution (Conan 2d20, Ancient Lineage perk) our bard player had to get pissed if his beauty, talent or the greatness of Brythunia were put into question (the perk only talks about the last part but he added the vain part)

Also he once got elected as "The King of Fools" in a Aquilonian town since he was considered the ugliest person in town by the villagers which broke him emotionally
