We don't know the true nature of the 'possession' or what even happened to be honest, I interpreted it that the real Nikki was trapped as a passenger in her body while the magic of the willow wish was piloting her mind, but she was still aware of what was happening in some way. Because the wish is for her to love Bear, whenever he isn't around it appears to cause her actual pain, like she said, or that could have been her just being manipulative in order to make him stay. Either way it seems like to fulfil the wish she has to be with him or else suffer in some way.
