so the plot is you play as the main guy and your girlfriend breaks up with you, one of your friends from high school dies of overdose, then your co-worker rapes you, and you quit your job because you don't want to be there around the guy who raped you, so now you're broke, and then your car breaks down on you, and the whole time this is happening there is a genocide taking place

I've thought for a while the nastiest thing you could do as a game dev would be to make an unassuming Stardew-style game where after you marry your waifu of choice, your relationship gradually gets worse and worse with nothing you can do about it until she divorces you, takes your farm and kids, and basically bricks your file.To execute properly on this plan roll it out as part of a content update after people have already bought the game and played it for 50 hours.

Through story it's easy. Just introduce likeable characters and murder them but give the player the idea that they could have saved them. Make sure to autosave so they can't try to savescum to find out it was impossible.
