The site does not need to rat you out to the ISP, your ISP knows what domain you visit, even over ssl, they just won't know the subdomain.
So they see a small amount of packets between you and the IP corresponding to the website being exchanged and a larger amount of packets being sent by the website to your IP, which means you are downloading something with a larger file size.
This is trivial to automatically recognize and send a notification by the ISP, they only need to realize the website hosts unwanted material and blacklist it.

Actually tracking a torrent without being connected to said torrent is nigh on impossible. The pieces come out of order from various IPs, and the infohash has to be captured (and be verified by the ISP).
Direct downloads can be somewhat monitored, but there's a lot of tech built into modern browsers that can obscure URLs from your ISP. The file itself also can be harder to fingerprint with everything using SSL these days.
Most of the time they just watch specific blacklisted sites.

For further clarification about the torrent thing, look at picrel.
Those are connected to a torrent I'm on. If I were a copyright holder, I'd sort out which of those aren't VPN IPs and contact the ISPs associated with them, saying they were downloading my copyrighted material at a specific time.
They would then check their logs to see who was provisioned said IP at that time, and contact said person (and possibly turn over names if I sent a subpoena for information).
The ISP itself is not monitoring torrent activity because it's not feasible.

Proton's a real one. Nord, Mullvad, and Express are also safe. I had issues with Express functionality when I used it a couple years back (it randomly swapped IPs and didn't support port forwarding), but it might be better now.
