never in any game it ever says that they hit themselves while confused, only that they hurt themselves in their confusion. Self inflicted damage can be many things, they could be tripping over, they could be missing their hits and crashing into things

I always imagined it was them toppling over, more like being dizzy and slamming into an object or thudding onto the floor than smacking themselves. And it kinda sorta makes sense that it would scale with attack that way. Falling over while your hands are knives would hurt more than if you're a squishy pink or transparent green blob you know?
