What can i do to not hate myself every waking moment of my life? everytime i speak or i do anything i feel like i'm doing it wrong, i've tried everything, i hit the gym, i went to therapy and i took medication for a time but the feeling ALWAYS comes back, so wtf do i do? kms?

I know that feeling well. What helps me the most, is to meditate daily on thoughts of gratitude. Every mistake that you make is an opportunity to improve. When you do something wrong, it doesn't create a deficiency in you it merely reveals a deficiency that was already there. If I have deficiencies of any kind I want to know about them so that I can work on them, so rather than feeling defeated, worthless or harmed by my imperfections I should always feel optimistic and thankful that my flaws have been highlighted so that I can begin the work of repairing them. It's better to actually be good than to merely look good, so if those issues had laid hidden I would be worse off. Every thought and every memory throughout the day of "I hate myself" or "I hate that I did that", should be immediately confronted with "No, I'm far more thankful that I learned not to do that". It's hard at first, but the more often you do it the easier it will get.

That's natural, repetition is a large part of learning. If you bump your head on a low doorframe 10 times before you finally begin to instinctively duck, then 10 was the number of bumps it was going to take. Each bump was necessary, the 1st bump was just as crucial as the 10th bump in the same way that a path isn't complete until every brick is laid. It would be nice if we always learnt from our mistakes on the first try, but it rarely works out that smoothly, so it shouldn't be our default expectation. The only thing that can really stop you from learning and improving is despair. When you stop trying grow and stop wanting to be better, that's when you'll stagnate and your failures will well and truly be failures rather than lessons. As long as you don't lose hope, you'll make it eventually.

i used to be on this hopium, but came to realize that solution to most mistakes i make is to just be all around less shit and less error prone. pretty daunting task to work at.

That confirms the idea, it doesn't reject it. You learned how to be less error prone due to your failures, which were lessons. If you hadn't failed, you wouldn't have learned to avoid errors. I'm not saying "be content to make mistakes and never improve". I'm saying "use what you've learned so that you may improve".

you probably just hit a negative spiral when you're alone. it'd be best to surround yourself with people as much as possible and keep yourself busy. somethings going on with your thoughts thats now creating a wall around you making you self-conscious rather than present in the moment. it will make interact with others difficult too since half your thoughts will be internalized and not in the moment.
you could go the self-acceptance route, but it seems like you've already tried a bit of that.
i'd start journaling, so you have something to reread to see what starts the self-hatred spiral.

Going through something similar to OP; just fucked up again recently, have been trying to not wallow and keep going. Your post is really helpful.

OP I want to say that the inner dialogue of self-hatred becomes a habit over time. I recommend finding the root of it - maybe your parents, siblings, or people from school, work, whatever. Whatever it was, the thoughts likely became a way for you to cope with the challenges and pain that life brings. I recommend digging deep to the root and finding self-compassion; try to have gratitude throughout each day. It's very hard to do when you hate yourself but it builds more positive connections in your brain. Try to not entertain the thoughts, basically. You deserve to exist without hating yourself
