I would apologize, leave everything clear and move somewhere else, so ghosts from your past can't come and fuck you up in your new place
if your reputation is only embarrassing and not criminal, people gets over it over time, you can even go back to the place you ran away from
also give time to any event and the gravitas of it diminishes tremendously, both for you but specially for the rest.
I did a lot of ridiculous things when I was in 1st year uni and I thought people would see me and point and laugh but I recently stumbled upon many of them and they were so happy to see me and didn't even remember those things, they were embarrassed by their own clownish stuff
