That's because a lot of manufacturers of extremely high heat hot sauce look only at the scoville units and will often time add capsaicin extract into the hot sauce to artificially increase the heat. Extracting capsaicin out of chilis also extracts other chemical compounds that are very bitter and when they add the capsaicin extract those bitter chemicals are added too. The raw chilis are actually very tasty, with Scorpions being very fruity and tropical, Carolina reapers are very citrussy, and ghost peppers are savory and meaty.

Habaneros clock it at around 100,000 scoville units. Ghosts clock in at 1,000,000. Reapers are 2,000,000. And the new pepper making the rounds ,the so called Pepper X, is supposed to be 2.5 million scoville units, though independent verification has not been performed. To make a really good hot sauce you do add a lot of garlic, but just garlic and peppers aren't good enough. You gotta balance all the flavors, you need onions, you need herbs, you need spices, you need something sweet to brighten it. If you want to go more earthy you add carrots, if not then red bell pepper. I made this hot sauce last year and it was phenomenal. This year I have proper fermentation jars with valves so I can really go for it.

The seeds of a chili carry a lot of capsaicin in them. Another thing that a lot of cheap hot sauce manufacturers will do is not remove the pith of the chilis. The pith contains a ton of capsaicin as well, but it is extremely bitter. To make a good hit sauce you remove the pith and most of the seeds of your chilis before fermentation.
