High Schools in the USA do a la carte stuff. You get certain choices with the card system, but if you have cash, you can just buy 40 servings of fries.
Salad, pizza, hot dogs, burgers, fried chicken, etc are the regulars; then they do like one special a day.

The US has over 20,000 high schools and several times more lower level schools that feed into the 20k high schools. Most schools participate in the USDA school lunch program, which sets certain nutritional guidelines school lunches must meet to be subsidized by the federal government. Outside of meeting those minimum standards, schools are mostly free to serve whatever they want, though individual state governments sometimes have additional rules the schools must follow.
Some of the better private schools opt out of the USDA program and have their own nutritionists calling the shots, but a surprisingly large percentage of even the expensive private schools can't resist the lure of free money, so they go by the USDA guidelines too. That's not necessarily terrible when the school and/or families are willing to add their own money to the pile. The main problem with the USDA guidelines is that they're carb heavy and the cheapest carbs tend to be quite terrible processed garbage. Schools trying to get by with just USDA funds end up providing terrible shit.
A good cook can plan out quality meals that meet the guidelines but that requires parents or the school to supplement the money available for meals. Most schools do not have good cooks or nutritionists, not do they have much money to spend on meals beyond the USDA funds. That's why you see photos of terrible trash so often. It's also why the Michelle Obama program, even with the extra funds it provided, caused so much trouble since the locals were already accustomed to certain trash sources and couldn't adapt to the new guidelines very well.
All of this can be avoided by parents who send their children to school with home prepped lunches.

This is the average lunch in PR, except once a week they make something lazy because they have to make inventory checks and write down the menu for the next week, so they prepare either hotdogs with mashed potatoes and sauerkraut, or ravioli with soda crackers. The daily fruit might be canned, or it might be fresh depending on the day. The juice in some schools comes in cartoons, but for mine it came in bags, it was either apple, pineapple, grape or fruit punch flavored. The milk always came in bags, unless you were among the few that wanted whole milk, in which case you had to grab a gallon and pour yourself a glass of it. For everyone else we drank milk that comes in bags, and depending on your school's food distributors you had chocolate milk, strawberry milk, vanilla milk, and recently banana milk. There are two major dairy corporations which supply schools, Suiza Dairy and Tres Monjitas, I prefer Suiza Dairy.

My least favorite menu was either the pre-made meatloaf cooked in a stew sauce, or sancocho stew (Stew made with tubers and pork)

Besides those offerings, there's also a permanent salad bar, the "quick pick" (where you could grab a piece of fruit, a bottled drink and 7-Eleven-tier wrapped sandwiches) and the deli line offers made to order subs besides the specialty sandwiches I posted here (Turkey BLTC Wrap; Balsamic Portobello Sandwich (Vg) and Roast Beef with Cheddar, Onions, & Horseradish; Avocado Egg Salad on 12 Grain Bread (V)). I usually went to the deli line.
