Has that even happened often enough to be trope? The Hobgoblin mystery was a pretty unique series of behind the scenes debacles where by that issue, Peter David was just trying to make the best out of the hand he'd been dealt. When has this ever happened again with another significant mystery box villain?

Some D-tier villains whose real identity never mattered dying and then a Handbook making up real names for them isn't the same and doesn't count.
