The body being naturally formed that way makes the form and the implications more interesting.That being said, I can get behind the idea of manual conjoinment when it's undistinguishable from natural conjoinment. Entities being magically conjoined or because of reality shifting shenanigans usually results in their body looking natural, so that would count.As for the background story, both the ideas of being recently conjoined and conjoined by birth have their perks. As long as what I wrote about is present. Though as I've mentioned, the former allows for interesting implications, especially when opposite (both genders being conjoined, as in picrel) or contradictory elements (one of the twin before the conjoinment is older by many years) are present.

It sucks that since this fetish is super niche, there aren’t really any artist to make conjoinment content. So it’s always great when a talented artist comes around and makes amazing conjoinment content. The only problem is these great artist are really rare to come by. And many of them don’t even make conjoined content anymore.
