Not quite as simple. I've got an 18" neck without apnea, but over 17" is considered a risk factor. Building muscle up into the obese range is where cardiovascular mortality starts going way up like with hams.

The heart can adapt, but the stress on the heart will eventually catch up, day after day, month after month.
Eventually your CNS will begin to slow down your body to ensure the heart can rest.
