I am someone in my mid to late 50s who also blasts gear and is relatively healthy, I have some ideas.

I do think O'Hearn has great genetics for this. Obviosuly. Without steroids, as a young man, he wouldve had a very, very good physique that was strong, lean and muscular. Steroids have allowed him to take it an extra 10-20% and to keep it this long.

However, he has also done everything right. He made it (his body and appearance) his profession and his life's work. He gets plastic surgery and some sort of hair treatment (plugs, surgery, grafts, or perhaps topicals like finasteride or minoxidil) to keep his hairline. Thats a big part of the looking healthy for him.

But also, he has trained consistently and reasonably heavy. He is strong. 405 for reps on incline is strong. If you watch his take on nutrition, its very balanced and "healthy" although he obviously biases a lot of protein. He never really let himself get fat or even bloated, and that makes a huge difference to skin, overall appearance and health.

He does train in a controleld, disciplined, consitent manner. Perhaps everything he cosshes to do is not "optimal" but I think he makes up for it by being extremely dedicated, disciplined, consitent and, once again, it has been his fucking JOB for the last 40 years to look like this. he took it seriosuly.

Finally, I am nowhere near as genetically blessed or look as good as OHearn. But, I as a man in my late 50s have bult an excellent physique using only TRT levels of testosterone, along with limited and judicious use of orals, ie "blasting and cruising". TRT with occasional anavar, winstrol or Dbol to gain some strength and size.

If I can do this (as a full time office worker with a wife and several kids and average genetics) it should be a cake walk for someone with excellent genetics, who is smart, who started out that way, who never got fat and makes it his job.

Necessary if you want to look "muscular". Probably not to be lean and healthy. That said, there are 1/1,000,000 genetics who if they did everything else right might be able to look lean and muscular without extra Test.

But taking TRT allows you a little slack in terms of doing everything else (food, rest, training) perfectly since its going to bias everything towards a bit more muscle growth and strength.

If youre in your forties and find yourself still looking shitty, even if youre doing everything else correct, you may as well take TRT becasue most men in thei day and age are low and if you take true TRT (like 100-200 milligrams er week) there are pretty much zero negative heslth effects. Even more (like up to 350-500) can have zero health effects, but some will start to see mild shit like LDL/HDL imbalances, elevated blood pressure etc.

If by the time you hit your 30s you're already consistently doing strength training, and keep it up, you won't NEED it till much later.
