Out of the gym (this is ideal), just find a long (ideally at least 600ft up) steep (like 25 degrees or steeper-- trails seldom get this steep) hill, climb it with >=25% of your bodyweight in a backpack. Stairmaster is worse but also works, treadmills don't get steep enough.Specificity matters, so if you can hike for your z2, do that. Hike with some added weight, emulate the objective you're training for, etc.The old fashioned advice is just "hike a lot" which tbf does work, the main problem is most people can't hike at the right intensity several times a week.Anyway, after you have a baseline of lower body and core strength, building more usually doesn't have any ROI. Bulking for strength gains is counterproductive here. Muscular endurance training becomes the focus.

if more fit people hike 20 miles, get stuck and exhausted, and don't face a rescue bill, then she shouldn't. I don't think the distance is the issue at hand, for someone to get exhausted in that manner takes probably a good day or even two to fully recover to be able to make that journey again to get out of there. So if that person is unprepared to camp out in the wilderness, it can become a serious situation. She's obviously too heavy to carry even 2 miles, but if we compare it to a fit person being stuck 20 miles out, it would be very hard to carry them too.
