Basically fully Free Software, open API and all features ever somebody does or wants (default is disabled). People can use what they want, how they want and where they want. One guy could use it as image board like version with tomorrow theme while other person would maybe use it on his RSS reader on iOS and third guy with Reddit alike UI on Android. Doesn't matter what you would want, it would be there. TikTok clone to browse videos with "comment section" or somebody would use whole site as a plugin in their application to import media and text.

Imagine if all image boards, forums, video and streaming sites would be like this and you could use all of it as a one website/application you like. Integrate it into local LLMs or thermostat, who knows.

Text board on I2P after the feds blow up the internet and people start actually using anonymization protocols, so people actually visit it.
Autolinking for IPFS and torrents because people are going to link to images anyway if it's a text board.
Captchas are 4chan-style obnoxious at the start but give you "no verification required" more often and you can pay in crypto to upgrade your anon identity pass-style.
Old, shitty phpBB smiley set, plus a few new ones in the same style, plus only the actually funny SA emotes.

Similar to r9k, a (non-remote) language model occasionally reads your posts and decides if you're have provided even a little informational value or entertainment value. It is extremely lenient on hostility and shitposting, as long as you don't do nothing but and your shitposts and hostility contain even a modicum of semantic value.
If you do nothing but, all your recent posts become collapsed by default. This, hopefully, creates an environment of trust where you can see someone call someone else a faggot or say something stupid and people don't immediately think "ugh, I bet this guy does nothing but call people faggots or say stupid things," and people feel the need to at least occasionally divulge interesting information and contribute to discussions so they can keep saying stupid things or calling people faggots, and feel discouraged from coming online in a bad mood just to yell at everyone.
There are a good few problems with this that I have both thought of and not thought of, but we're assuming I could design the perfect BBS so it'd just work in this ideal world.

A dozen or so paid human volunteers that help tune the above model and deal with immediate problems.
Despite it being a text board, hidden board with a dedicated avatarfag mood avatar system you can upload custom avatar sets for yourself to, in an attempt to create superautism in a controlled environment just for fun.

Also lurking lowers initial captcha difficulty. Not to a severe degree, it'd be more like "if you visit the site and try to post within 2 minutes of opening it you get an Arkose Labs style bullshit challenge plus a PoW, if you actively browse the site for a few hours then get a captcha it's obnoxious but reasonable"
The idea is to create trustable pseudonymous identities that the user does not want to lose without requiring a signup, as the internet has thoroughly proven you cannot just slap a post form with a normal captcha on your site and hope for the best when you have any reasonably sized userbase, and I have zero faith in humanity anymore.

I think a lot of people underestimate the fact that making the site harder to use can be good. You need to boil the frog by not rolling out and turning up every bullshit measure until you're maybe even years in so you can accrue enough regulars, but once you have a big enough circlejerk with a modicum of friction to join and the promise of being able to discuss high-value topics, you've basically reinvented the Masonic lodge (a historically winning formula) with the only issue being that there isn't a captcha for likemindedness. Except for that one that makes you name anime girls based on their pictures.

I'm not a big fan of the idea of having to pay to post. I am fine with paying fast tracking you past the anti-abuse systems and making the experience better, but even SomethingAwful waited 2 years before implementing the "you must pay" system, which reinforces what I said about needing initial lack of friction to gain a grandfathered-in userbase. Nobody's going to pay for a site that nobody's already on.
