This is a good starting point and sets sane defaults for most desktops and laptops that aren't potatoes.
If you want actual tuning, it depends on what you're watching, what your screen resolution is, and if you want to change your monitor's refresh rate for the content or use interpolation(don't).

Tuning can get rather detailed depending on personal preference, system specs, if you're upscaling or downscaling, how much you're upscaling by, etc.

It is the opposite, once you have setup your conf file it gets set in stone and you never touch it again, at least that's the case for me.

The config is there for you to tailor it to specific needs or taste. You can make things look better with some meme shaders, but it's generally not worth the compute unless you have a lot of compute to spare and care a lot about a relatively small difference.
