If it's from damaged sensory hairs in the cochlea, it's a valid sensory signal going to the brain, and it's no more mental than the pain you feel in a wound. If it's from damage to your auditory nerve, it's more like the tingles and pains that a damaged nerve elsewhere can cause. A damaged auditory cortex can also cause tinnitus though, which is mental but not a psychological problem.
