I have bought many used CDs this year and have some of my old ones. Only one exhibited this kind of issue. It's mostly a meme but bad pressings from specific plants can have this problem. There was a Coldcut album where it took multiple people on the torrent tracker redacted trying to rip their copies until someone got it to read perfectly. Parity data from cuetools helps with this issue.

It highly depends, CD-DAO or write-on-demand mode where you slowly fill a disc is very dodgy, burning an ISO full of files if there is damage, it might be confined to a small amount of data even within a single file.
