It was always going to be a problem, but the Spanish made it FAR, FAR worse by depriving natives of proper food and medicine, and their slave trade caused lots of sick people to be rapidly shipped around and forced into extremely close quarters with each other. The natives who weren't in contact with the Spanish would have still been fucked up when the diseases hit them, but the diseases would have spread to them far slower and more gradually if the Spaniards hadn't turned them into a nightmarish giga pandemic in Mexico, so they would have been able to react to them much more effectively.It's hard to estimate exact numbers, but our world's situation of the population of the Americas dropping 80-95% within one century and some regions like the Amazon losing literally all of urban civilization and devolving into borderline cavemen really seems like a worst case scenario
