What they do is they photoshop or sometimes even AI generate a woman/women seen in the video, then put it in the thumbnail while frontloading the one or two attractive women in the first 10 seconds of video. After that the video will be completely normal, not very creepy, and there's barely anyone is worth noting as being particularly attractive.
