he was 31, unmarried and childless. i never met him.

So the guy was severely autistic and had epilepsy, his carer was 60 and it was november. Not that big of a mystery. He probably fell in, the carer jumped in after him and couldn't get him out before hypothermia got them both. This doesn't look like an easy spot for an exhausted old man to climb out of with a freaking out autist in tow even if the water isn't deep.
