Even the idea of a MAD scenario is pithy compared to the mental image of that era. Think of all the strike missions carried out across the Second World War: every bomber raid, every fire mission, every P-47 marauding its way down German highways, and replace that with a nuclear weapon. That was the world they were staring down the barrel of. WW2 was fought in every corner of the globe for half a decade, and nuclear weapons were the obvious way to try and fix the problem of lugging around enough ammunition to actually conduct a war on that scale. Today, we have the luxury of sitting in a world where nukes didn't get cheap enough to spam them on that scale, the PGM and JDAM fill in everything you could need on the tactical and industrial targeting side, and most importantly, Harry Truman balked at the idea of pushing the button a second time.

I doubt someone would waste nukes on Switzerland, in fact, there are plenty of places that could survive, especially if this WW3 scenario isn't between anyone and everyone. It would be a limited exchange even if all nukes are launched on both sides, they wouldn't waste nuclear weapons on random nonparticipating countries. The only real concern would be food security, but I doubt it would be a big problem for actual human beings (e.g. no africans, indians, or mid easterners). Some places would probably be completely fine, such as South America, and would only have to grapple with food scarcity instead of massive rebuilding.
