Bro, the cylinders of a cat are housed internally.
Cylinder and Barrel gap exist due to the cylinder need to "revolve" to position the next round.
You want a "gapless" design, you will a nagant type mechanism.

If you do a top break, it should actually be conceivably easier to maintain extremely tight cylinder gap tolerances, because the cylinder no longer has to swing out on a crane, only rotate.
So if your cylinder gap is extremely tight, you should be able to minimize gas escape and thus reduce noise a significant amount, provided you've also put a suppressor on the barrel.
It's not going to be anywhere near as quiet as a standard sealed chamber, but might be tolerable to the open ear.

I think you could machine a cylinder to have those extension from the chambers instead of the casings. wouldn't seal up as tight as expanded brass(unless you spent korth amounts of money on each gun) but would eliminate the foreskin requirement

5.7x28mm is kind of dogshit for suppressing because at most you're just reigning in the muzzleblast, the projectile is still way above the sound barrier.
It's also a dogshit choice for a revolver, since high pressured bottle-necked cartridges are very prone to setting back and binding up the action.

People have made SCHV revolvers before, and bottle-necked cartridges cause a lot of problems on that point. It's why the Smith & Wesson Model 53 was a flop.

You can't do that with conventional 5.7x28mm ammunition, you need a cartridge case which actually does this, which is why the ammunition for the Russian Nagant revolver, 7.62x38mmRG, looks like it does.

At that point, you might as well put replaceable rubber gaskets around the mouths of the chambers. You're not getting around case setback that way though.

If you want a high pressured bottle-necked cartridge in a revolver, this approach is pretty much your only solution, the .357-44 Bobcat Magnum

This is .44 Magnum necked down to .357 caliber, with a short .44 caliber chamber, with a normal .357 caliber barrel, and the case then has a spacer sleeve over its neck and shoulder to prevent it from setting back and binding. This works, but the rings/sleeves need to be epoxied onto the case, and they wear out from use.

You will also want to stick to 180gr bullets or heavier, as otherwise you'll see the same kind of top-strap erosion issues as when people loaded light bullets for muh velocity with .357 Maximum. Though, for a hypothetical revolver which would use a sealing action and uncircumcised cases, that should not be a problem.

For what OP wants to do, he needs a special version of 5.7mm which has the extended neck and also probably a partially extended spacer sleeve glued onto it to cover its neck.

It will still be a very loud gun, however.
