you see someone whos overweight they are inherently more attractive. It activates the primal part of your brain that goes "omg this person can eat well they get all the nutrients they need and more, must breed must breed must breed" it's inherently really hot. But when you see someone underweight it's this visceral gross out reaction unless you are anorexic as well. It's like this visceral repulsion that makes you immediately freak out "oh ew wtf is wrong with that person get away from me" and activate your fight or flight response, it's like seeing a dog with rabies it's like this mix of feeling bad on their behalf and feeling grossed out like you need to leave immediately.
