Typically the person is still taking measures to transition medically such as going on hormone therapy and permanent facial hair removal, hence why the common depiction involves wearing baggy clothes to hide it.IIRC the term comes from how some trans women have only partly socially transitioned in certain situations, with ones where they can present as female being referred to being in "girl mode" while situations where they have to present as male are "boy mode".
