Cells digest cells, and the skin in there is still semi-permeable (as all skin is). If the microbiome is maintained properly, it should not need to be cleaned externally.

Cis vaginas are not self-cleaning just because they are attached to something inside; it's because of the microbiome living there that digests material. The problem is that the skin post-bottom-surgery in trans women still does not secrete the material a cis-equivalent microbiome needs to live properly, so just introducing it won't allow it to survive.

It is a problem some cis women have too, without knowing about it. A healthy vaginal microbiome will deal with most issues involved in sex or dead cells. It also helps prevent yeast infections and other infections.

Just like for cis women, washing it out will harm the microbiome. Unlike for cis women, repairing the microbiome means restarting largely from scratch.

Washing in there daily is another option, but it seems far less pleasant.
