I've got diagnosed autism and I agree. The explosion of autism is just because we let women into the workplace and especially HRM who will refuse to hire you because you're "creepy" but they'll hire some social retard because he's "le extraverted". I keep hearing about this. It's when you get paid for your condition or whatever, right? Since I was diagnosed as a kid I was never really told anything, I assume that's just the way it goes though, explain to the parents and not the kid. I'm 22 now but I've never received any aid for my autism and I haven't really seeked it out either, I wouldn't know the first thing about applying for it. Well I got it, but autism wasn't the reason (I was actually only diagnosed with autism years after the fact) so I can't really help you with this. I also live in Europe and things might be handled differently here than in the US (though I've heard many areas over there do have their own form of autism bux). In general though I wouldn't recommend it if you're reasonably functioning right now. The prospect of not having to work sounds nice on paper but after many years you'll just become a social outcast with no friends or acquaintances and such a large distance to the workplace you'll never be able to live a semblance of a normal life any longer. Also, therapy is useless, wasted over 10 years on that shit and am only now finally rid of medication that has caused permanent side effects I'm still trying to mitigate. If you do anything, at least make sure you don't take any psychiatrist-prescribed pills and if you absolutely HAVE to, schedule with the lead practitioner to evaluate their necessity after one year. If you can get through the day without them, don't take them.

I don't know how even noticeable autism shares the same mechanism as the second example. I just know that when I'm more or less directed to just accept a thing as fact and move on, that there are important details being omitted. My suspicion is that there is a sizable overlap between autism and intellectual disability that goes unspoken of because no one wants to seem insensitive.
I've already seen it once. In my job, you often see kids grow up and their behavior change as they mature. I've seen two siblings from middle school through high school, and one has matured and changed. The other is a HS senior and other than his voice dropping, he acts little different to when I first met him in 6th grade. I know that he has an intellectual disability, but it's never been diagnosed. I've asked questions about this and have basically been told to shut up.
Don't attempt a diagnosis, and don't refer out for a diagnosis. If you indicate anything about it in your notes, then INSURANCE might come down on you like a hammer.

Your examples are not representative of the majority of people diagnosed with autism nowadays though. For me, I didn't get diagnosed until I was nearly 30 because my first diagnosis (using older methodology) literally said "You CANNOT be autistic, because you are capable of empathy and autists are incapable of empathy." I had to look at drawings where some scene was taking place, such as a man crying at a bed where a woman was lying, either dead or asleep it wasn't clear (purposely), and because I could make suggestions for what was taking place (like perhaps the man is coming home to his wife at night and is overcome with grief because he just cheated, or he just found his wife has passed away, things like that) that meant I could not be autistic. But then later when they "revised" their methodology, I'm suddenly autistic. Huh. I'm not denying it or anything, because it's probably correct, but for cases like myself, telling people they're incapable of participating in the workplace because of that just creates swaths of capable people sitting at home doing nothing because Jane thinks Anon is kinda weird and a bummer how he doesn't like the New Year's reception.

To be fair we know very VERY little on that front. It's a very complicated condition that's so high level brain wise that pin pointing it onto anything is as tough as it goes.
So as always everything is on the table, genetics, chemical imbalance, synaptic developmental problems. (https://doi.org/10.1016/j.tins.2020.03.005). And of course we have models for it but again, as you pointed out, we just don't know what can influence it to be more thomas the tank engine sided or full on potato.

I'm a diagnosed autist and a bit of a psych geek, and the best explanation I've heard for autism on both a personal and scientific level is that the autistic brain makes a whole shitload of extra pathways between neurons, meaning that any stimulus to the brain is amplified, and will stimulate regions of the brain that shouldn't be connected at all. In low amounts, this can result in increased creativity and/or pattern recognition, because the autistic mind is constantly making connections between extremely unrelated topics. This is why autistic fandoms tend to produce weird shit like MLP/Fallout crossovers.

The trade-off is a lack of focus because all incoming stimulus is going to be far more intense. This is why autists need routine. Most people would go crazy if they ate the same foods, wore the same clothes, did the exact same things in the same order every single day. For autists, this sort of life is comforting, because anything in their life that's unpredictable hits them like a truck. Imagine if every time you heard a loud noise you also experienced a bright light shining in your face, as well as an electric shock on your skin. It's too much information for the brain to process. Familiarity mitigates the risk of getting overloaded with stimuli. It makes the few unpredictable things in life much more manageable.

Conversation is especially overwhelming. Our brains are designed to pay very close attention to how other people communicate. Body language, phrasing, tone of voice, facial expressions, etc. Autistics are famous for being oblivious to a lot of that stuff, but it's more accurate to say they can only deal with a bit of information at a time. Not making eye contact is a way of shutting out all the overwhelming information that comes with looking at someone directly as they're communicating with you. This is also why autists often prefer talking to people online. It's like taking a television that's playing every channel at once and shutting all of them off but one. It sounds like your Patient A was overwhelmed when others tried to talk to him. With the same person speaking to him consistently over a long period of time, that introduced predictability instead of chaos. He adopted your speech patterns because your 'signal' was the easiest to decipher.

Patient B is obviously a more severe case, but you can still see the same basic logic at work. Something makes Patient B upset. That stimulus that should be hitting a part of the brain labeled 'cry' goes through neural pathways that shouldn't be there and arrives at 'laugh' instead. Inappropriate laughter is surprisingly common among autistics. I'd guess that his screaming is always in response to something happening, but when even the smallest stimulus can be cripplingly overwhelming, any tiny random event could be the cause. Hurting himself is likely a way to block out all that stimuli, like getting blackout drunk or high.
