It's not uncommon to asphyxiate from gasses inside an in use septic tank. It happened many times. But if it was just fresh tank under construction then what gives? Was it full of some gasses from chemicals they used during construction?

I've worked factory jobs and been told there was some policy against climbing ladders and walking the scaffolding, which as it turns out was not true. Seriously though, these kinds of spaces typically have twists and turns and pieces sticking out and that kind of thing. If you just tie a rope around some guy's waste it won't help very much. He will most likely get stuck on something as you drag him and smash into shit and if you're luck you can break his arms or whatever to work him around or through the obstacle. But if he's not wearing a proper harness the rope will just fall off his mangled body at that point anyway.
It is very rare that you have an unobstructed path to where you are going and can just lift someone directly out of a space. Don't fuck around with this stuff.

The danger is often invisible and odorless. It may not even be clear if the guy is having a heart attack or what exactly is going on. If you haven't been taught about the danger or seen it first hand yourself before then you will not know about it.
Also, even using people to test in stages like you suggest is still incredibly dangerous. It may take some time for you to feel weak or things may get worse over time.
For example while observing the guy at stage 4 from stage 3 you may become too weak to even return to the guy at stage 2. Now some guy has to drag your ass back assuming he doesn't become too weak himself. and the guy who collapsed at stage 4 is going to die because there isn't enough oxygen at stage 3 for you to be strong enough to pull him back.
7000+ years ago they didn't really give too many fucks if a miner or two died.
