In my state, A is the only legal answer that is always legal, D is only legal if you clock in IMMEDIATELY and are paid for your entire would-have-been break. If you are offered a break by your employer, you are completely relieved from duty until the conclusion of your break. No exceptions. Even passive tasks like greeting a guest, opening a door, or answering a phone count as work.
The employer legally can't deduct the time they spent eating before they were called back to work, either. However, if the break would have been under 20 minutes, it counts as a short rest break rather than a meal break. What happens now is the manager has to individually correct every single timecard for everyone who was on break so they are paid continuously from the time they would have clocked out. And then the district manager writes up the general manager because he just let the entire store sit for a break and get paid for it.
B is completely illegal. High risk of audit.
C is double plus completely illegal. Critical audit risk and total disaster in a lawsuit. If such a lawsuit happens, the company is completely responsible for the approximately $100,000 worth of legal fees, and there is exactly a 0.000% chance of winning the case. Needless to say, the manager responsible would be immediately terminated.

I remember working grill one time cooking bacon. The first manager said I was undercooking the bacon and need to cook it more. The next day a second manager told me i was over cooking the bacon and I need to not cook it as much. A bit later the main grill guy arrived and before taking over the station told me that I was cooking it perfectly and said it doesnt matter what I do they're going to find something to criticize you for.
