Thats unusual, maybe a control surface got stuck in the wrong spot or a catastrophic airframe failure? Either way the short timeframe and G-load/attitude confusion would make bailing out tricky.

Finland had similar case in 2014 where wing failed and only 3 jumpers made it out of plane while 9 died. It was even in 4km altitude but failed wing blocked the exit door. Flat spin can also prevent exit due to high g-forces.
