I've been to japan during the july heat waves and it's miserable outside. Luckily I had a car so I just drove it while blasting the AC and barely walked outside. It was a common sight to see cars just running with AC on in the parking lots while the drivers were shojpping because it's just so damn hot and humid.

They have tropical huminidty, went there last July, it was "only" 32°C but we were soaked in minutes outsides, and it was even hard to breath for the first few days.
At 40°C I imagine the problem is the thermal shock from going from a 18°C air conditionned inside to a few minutes in the Sauna of the streets. Japan put their AC way too low imo, where you feel cold in summer cloth.
