Hospitals make their profits by extracting as much money from health insurance companies as possible. This will lead to them sending insurance companies comically overpriced invoices and then negotiating down until both parties are satisfied.
Normally this is fine and the average person never sees any of it. But it breaks down completely if you don't have insurance for one reason or another. Like if you're between jobs or you haven't gotten around to applying to Medicaid, or the big one is if the insurance company has figured out a legal technicality that allows them to tell you to fuck off.

The main problem is Medicaid is only available for a single person if they make less than like $15,000 a year, so if you're making $20,000 a year you're still making poverty money but you also need to either pay $500 a month for health insurance. Medicaid pays for anything, it's better than any private healthcare plan that a normal person can pay for with money. Hospital administrator's ears perk up when they hear medicaid because of how much money they're about to make off you. You lose Medicaid after making like $23k annually depending on the state. At that point, your cheapest option for any kind of health insurance is about $500 a month (like 1/3 of your take-home pay) but it doesn't cover any actual medical expenses until you're spending another $14,000 in healthcare that year. So you're paying out of pocket for everything you actually need unless you're chronically ill and need medication to live, in which case it's saving you a few bucks over just buying the medication outright. This applies until you're spending like $800-1000 a month, per person, on health insurance, at which point the deductible goes down to where they actually pay for the medical treatment a normal person might need. But at that point you're paying $12,000 a year in insurance, which is more than you would have spent on normal medical treatment in a year. For example, I had a colonoscopy and esophageal endoscopy earlier this year and it was $5000 in the hospital, I paid list price out of pocket to the hospital but I still pay the $500 a month in health insurance because I haven't spent $14,000 yet this year at the hospital. If you make less than "upper class" wages, or if you have kids, this is a ridiculous amount of money to blow on the remote possibility of a debilitating illness or injury. It's engineered to support the chronically ill as a lowest common denominator, and creates an environment where people put off routine care and wind up chronically ill or spending millions on heroic treatment when nature's bill comes due.

Emergency departments in the US are required to treat you, even if you can't pay. Last year I was in a serious car accident that included abdominal surgery, multi-day ICU stay, and an air ambulance. I'd changed to a smaller company which didn't have the clout to swing a so-called Cadillac Plan so the coverage wasn't as good but I'm still only in for the $5K deductible and a crummy one-mile ride in an ambulance for $1200. Again, not one lawyer involved by me.

The real failing of US-style health insurance is tying it to employers. That's on purpose of course

I'm currently on Medicaid, which is overall nice in my blue state, but my eye doctor won't accept it with the network that administers it in my state for me, so we have to pay out-of-pocket for that.

So if you can't afford it you basically get state-funded healthcare and honestly it's pretty good depending ok what state you're in, like I was able to get my tonsils removed for like 50 bucks for the entire process and aftercare instead of paying 5,000 bucks out of pocket for it on my state sponsored thing. If you're making over like 100k you can usually get something pretty solid that it's fine and you're taken care of if it's not through your employer.
However, if you make say between 40-50k and 100k then however you get fucked in the ass by the insurance company where they basically don't cover shit and you don't get any breaks either.

It was better when like the knights of Columbus or the rotary were able to offer health insurance and work things out better in the past.

Last year I stubbed my big toe badly and it got swollen. I suspected it was sprained so I just let it heal. After 8 months it wasn't healing so I made an appointment with my podiatrist. I got an appointment within a week, went in, he saw it, took an x-ray, said "you have early onset arthritis, we'll give you a cortisone shot now and every few months and see what happens," he then got the shot, injected it, and I was out the door within an hour. I had no copay. That was a month ago and the swelling went down and pain went away. Boring stories like that don't sell a narrative.
