Using a boiling water reactor as an example because it's simpler.Reactor turns water into steam which spins a turbine. This water is then cooled by colder water which condenses it back into liquid to be sent back into the reactor. The water that condenses the reactor water is then warm and they need cold water so they take the warm water and dispose of it either through a cooling tower where some is recondensed and the rest is released to the atmosphere or the water is just dumped into a lake or river or ocean to let mother nature take its course. To replace this water more water needs to be taken in, generally from the same body of water that they dumped the warm water into.If water levels are too low the intakes cannot take new water to cool the reactor water meaning it can't recondense, steam will build up, and the reactor itself will melt down. Put simply in the industry the body of water you draw water from is called the ultimate heat sink, because without that the plant, even with all its safety measures can and will melt down.
