Is some of it government, but also some of it cultural. Even in the US, people from rural communities stimatize being depressed or serious mental illness. There definitely are schizophrenic, demented, and insane people who live in small towns, especially since they skew older in age.Also, each country has their own DSM or criterion by which they diagnosis and evaluate mental illness.

This is an advantages for anyone, regardless if you have anxiety or not, you don't even have to lie to get an official diagnosis of anxiety, you can just "lean into" any sort of feeling of anxiety and express that feeling. "yeah, sometimes i feel anxious when i'm around other people, or when I walk into a room and there is a lot of people there". A pretty normal experience, but one that is hard to grade, so the line between debilitating anxiety and normal human experience is pretty blurry.
