The only thing intercepting a ballistic missile over the ocean is an interceptor missile from a destroyer or something, and the destroyers won't get close enough to these ship to ship transfers so the tankers are effectively defenseless against ballistic missiles.
This could just signal that the US navy figured out a way to protect enough against drones, mines, and speed boats to create an effective bridge with these ship to ship transfers.
Iran just needs to blow up any ship that moves in or out of the hormuz and stop playing nice.

I think you wouldn't want to waste ballistic missiles on tanker ships if you can blow them up with cheap drones or hijack them with speed boats.
I'd only use ballistics on tankers for testing or if I didn't have another option. Or maybe like someone mentioned, Iran wants them to go boom and sink to the bottom of the sea to send a message.
So my thought is that since the US navy won't send destroyers close enough to escort tankers, they lack ballistic missile protection but are getting air cover (jets, helicopters, etc.) and that has been shooting down enough drones for transfers to be safe.
It's not an economical battle ($400-500k for a sidewiner or $1M + for an AMRAAM vs each $20k iranian drone), but it does protect the tankers if your only threats to them are slow moving drones that you can pick off.
